Preparing for your appointment
A referral from your general practitioner or another specialist to Dr Costello is required. Upon receipt of the referral (usually faxed) Dr Costello will triage it and the practice staff will contact you to arrange a suitable appointment. If you have not heard from us within a week, please phone 07 4725 0042 to make your appointment. Referrals also provide you eligibility for a Medicare rebate. If your bank account details are registered with Medicare we will be able to submit your claim for this rebate on-line when the account is finalised at completion of your appointment.
It is most important to have up to date details of your telephone number on the referral letter, particularly mobile number. We do not arrange appointments by email correspondence.
When arranging your appointment the reception staff will discuss billing procedures with you, including costs and Medicare rebates. Payment is required on the day of consultation, unless you have made arrangements with an outside entity such as WorkCover or an Insurance Company.

The Headache Diary is for you to keep a record of how your headaches are affecting you. Please bring your diaries with you to your next appointment with Dr Costello.

Cancellation Policy
Whilst we understand circumstances change and sometimes appointments cannot be kept, if you are unable to attend your appointment please phone and advise us. If a patient fails to attend and does not advise the reception staff, you may incur a cancellation fee.
Emergency
If urgent medical assistance is required please contact your local emergency services or present to your local emergency department.
